Quinceañera Booking Agreement

I understand that I need to submit records for the quinceañera's baptism, first communion, and confirmation in order for this booking to be complete.

I understand that the quinceañera, along with her parents or guardians need to attend a quinceañera preparation day retreat within 12 months of of the quinceañera mass and failure to do so may result in the quinceañera being cancelled.

​I understand that ​the quinceañera must participate in the IHM youth group or another parish ministry for at least 6 months prior to the quinceañera mass, and that failure to meet this requirement may result in ​the quinceañera being cancelled.

I understand that we must arrive at the church early and be ready to begin mass at the booked time. If we are late, the quinceañera may not be able to proceed as there may be other events scheduled after the quinceañera.
